Our structured patient pathway ensures that you receive the appropriate care at the right time, optimizing recovery and improving quality of life. Below is an overview of what you can expect during your journey with us.

  • Your current provider directly refers you to Infini Health. Our triage specialists will review the referral to assess urgency and assign a specialist based on priority. This ensures that patients with more severe or urgent conditions receive timely care, while all patients are matched with the most appropriate provider for their needs.

  • Once your referral has been processed, you will meet with a specialist for a comprehensive assessment. The specific evaluations conducted during this step will vary based on your condition, but generally include:

    Medical History Review – Discussion of symptoms, duration, previous treatments, comorbidities, and impact on daily activities.

    Physical Examination – Assessment of movement, function, pain levels, and any condition-specific tests. For example, joint stability assessments for osteoarthritis, range of motion tests for frozen shoulder, or neurological evaluations for sciatica.

    Diagnostic Imaging & Reports – If imaging (such as X-rays, MRIs, or ultrasounds) has not already been conducted, it may be ordered to confirm the diagnosis and guide treatment planning.

  • Based on the findings from the assessment, your specialist will discuss available treatment options and create a personalized treatment plan. This plan is designed according to your condition’s progression, severity, and individual health factors.

    Condition-Specific Approaches:

    For frozen shoulder, treatment may focus on managing pain and gradually improving mobility through guided physiotherapy.

    For sciatica, interventions might prioritize reducing nerve irritation, improving posture, and strengthening supporting muscles.

    For osteoarthritis, strategies may emphasize pain management, joint preservation, and maintaining functionality to delay or avoid surgical intervention.

  • Continued Care:

    Essential for ensuring progress and making adjustments as needed. Follow-up schedules and monitoring methods will differ depending on the condition being treated:

    • Short-Term Follow-Ups: Regularly scheduled visits to track early response to treatment, adjust interventions if necessary, and provide additional guidance.
