Our Referral Process:
Please note: No referrals are needed for Physiotherapy appointment requests. You may book these services directly over the phone.
For Patients:
At Infini Health, assessments and procedures for pain management are available by physician referral only. If you require treatment, please speak with your family doctor or primary care provider. They will complete the necessary Referral Form to initiate your care with us. If you would like to easily understand if you are a candidate for our services, please click here.
For Physicians:
Thank you for trusting Infini Health with your patients' care. To ensure a smooth transition and timely access to consultations and interventional pain procedures, please complete and send the Requisition Forms to our clinic. Referrals help us streamline communication and deliver the right treatment plan for your patients.

Discover if You Qualify for Our Services Below
Please read the entire list below. If any of these situations apply to you, it’s best to speak with your family doctor first. They can help figure out the kind of care or specialist that is the right fit for your symptoms.
You have had surgery on your back or spine in the last 6 months. This includes any type of operation on your spine, neck, or lower back that was done within the past 6 months.
You have an infection in your bones or spine right now. This includes known medical conditions like discitis or osteomyelitis, where there is an infection in the spinal bones or nearby tissues.
You have a bleeding condition or take medications that affect your blood clotting. This includes blood thinners such as warfarin, apixaban, or others, especially if these medications cannot be changed or paused.
You have pain all over your body or pain that is not caused by movement or injury. This includes conditions like fibromyalgia or other types of pain that don’t get worse or better with activity or rest.
You have cancer or pain related to cancer. This means you are currently living with cancer, or you have pain that is caused by cancer in any part of your body.
You are pregnant or planning to become pregnant soon. This includes if you are currently pregnant, think you might be, or are trying to become pregnant in the near future.
You have a mental health condition that is not being treated. This includes conditions such as schizophrenia, severe depression, bipolar disorder, or hearing or seeing things that others don’t, if you are not currently receiving care or treatment for it.
You have a problem with drug or alcohol use that is not under control. This means you regularly use drugs or alcohol that affects your daily life or health, and you’re not currently in a treatment or recovery program.

If none of the criteria above applied to you, we now ask that you complete a short pre-screening form. It takes just a few minutes to fill out and asks about your pain symptoms, recent treatments, and any medical reasons you may qualify for our services.
